TGA Medical Cart Request Form

The Medical Cart Request Form is currently experiencing technical difficulties and is not allowing submissions to upload any of the required documents. Please email Chris Untiedt directly at [email protected] with your information and the three (3) documents listed below.

This completed Medical Cart Request Form together with the required supporting documents must be submitted to the TGA NO LESS THAN FIVE (5) DAYS of the championship or event date for which the Medical Cart is being requested. Incomplete or late submissions may result in insufficient time to evaluate. All submissions are confidential and reviewed soley for the purpose of evaluating the medical cart request.

If the player/caddie/spectator has previously executed a stipulation governing the use of a golf cart during a TGA champioship or event within the same calendar year, the applicant is not required to submit the above referenced form and medical support documents. The previously executed stipulation may govern the use of the golf cart if the intended use is in the same capacity. In all cases relating to previously executed Stipulations, the person requesting the medical cart MUST contact TGA Senior Tournament Director Chris Untiedt ([email protected]) and must provide a copy of the Stipulation NO LESS THAN FIVE DAYS of the championship or event date.

The player/caddie/spectator must receive confirmation of such request from the TGA Championship department to ensure that a cart will be available at the specific championship or event. It is the responsibility of the person requesting the cart to follow up with the Championship department to obtain the acknowledged receipt to ensure that a cart will be available.
